Rivergate Sports Permission Forms

Below you will find downloadable forms that tell about the sports program and allow you to give permission for your child to play sports at Rivergate.  If you prefer, you may fill out the online form below and email it to Mr. Patchin giving permission for your child to play sports at Rivergate.

Please don't forget the Sports Fee.  Please turn it in to Mr. Patchin or the front office.
